HealthValues, LLC (“HealthValues”) contracts with one or more third parties to provide you with the products, services and benefits of your membership. In these Terms of Membership, the terms “we”, “our” and “us” refer to HealthValues. The terms “you” and “your” refer to the HealthValues member.
We look forward to serving you as a HealthValues member. Your membership and the privileges and conditions of membership are described below. A HealthValues Membership is available to all qualifying individuals 18 years of age and over. We reserve the right to refuse membership to any applicant and membership is revocable without cause. Membership is subject to any and all rules adopted by HealthValues including our Terms of Service, Privacy Policies and practices, and they may be amended from time to time without notice.
Your HealthValues Membership starts on the day after enrollment by the primary account holder and continues on a month-to-month basis from such date until terminated. For example, if the primary account holder enrolls on March 23, 2016 their HealthValues Membership begins on March 24, 2016 and renews each month on or about the same date (adjustments will be made for enrollments that start on date which does not recur each calendar month).
You will be required to provide your membership ID and/or other personal information to receive the products and services included in your HealthValues Membership.
Memberships may be terminated at HealthValue’s discretion.
Memberships are not transferable without HealthValue’s permission. Notwithstanding the foregoing, HealthValue’s may, at its discretion, authorize the transfer of the membership of a deceased owner to his or her surviving spouse, domestic partner, or immediate family member who is living at the same address, upon the written request of such surviving individual by sending an email to email@example.com.
The primary member must authorize additions or deletions of eligible family members, and is responsible for the membership.
Primary Members may add eligible family members to their membership for an additional fee. Eligible family members may include the primary member’s spouse, domestic partner, or immediate family members and living at the same address. To add eligible family members to your HealthValues Membership, please contact our member services personnel at firstname.lastname@example.org, call our toll-free number at 888-635-0292.
Primary members are responsible for the family members on their membership.
The benefits of your HealthValues Membership are offered at the sole discretion of HealthValues and may vary by availability, vendor and member’s state of residence. Our products and services are provided by third parties who reserve the right to withdraw without notice. HealthValues makes no affirmation of fact or promise relating to the products and services reflected. When you are using a vendor’s product or service, the information you provide to the vendor is not provided to HealthValues. You agree to hold HealthValues harmless from claim for damages you may have with any vendor. Unless otherwise stated, all offers are subject to change. Only members in good standing, with dues paid current, are entitled to receive member benefits. Your membership automatically expires if not paid on the due date.
We welcome debit/ATM cards, Credit Cards, and Paypal accounts.
A one-time set-up fee applies at the time of registration for a HealthValues Membership.
Payment of monthly membership fees will begin on the date you enroll for a HealthValues Membership and will be due in advance each month on or about the same day each month. Memberships are auto-billed each month; your membership fees will be charged to you automatically on the debit/ATM cards, credit cards or Paypal account you provided at the time of enrollment.
You are responsible for keeping your account information up-to-date. If any legal action is brought by or on behalf of HealthValues to collect payment, the primary member will be liable for reasonable fees and costs of collection.
You are responsible for notifying us and terminating your HealthValues Membership if you no longer wish to continue your HealthValues Membership.
Your HealthValues Membership may be terminated upon request of the primary member by sending an email to email@example.com or by calling 888-635-0292. Termination will be effective at the end of the current membership month for which payment was received.
Your HealthValues Membership will be automatically terminated if payment is not received.
Refunds of membership fees are not provided for active HealthValues Memberships.
Any refund of membership fees is at the sole discretion of HealthValues and will not exceed two months of membership fees paid from the date of the request for refund.
